If your organization is state tax exempt, you must submit a completed IRS 3372 form prior to arrival.
Email the completed form to scrreservations@shantycreek.com (or it fax to 231-533-2050).
Be sure to note the name of the person on the lodging reservation associated with this form in your email or fax.
KNOW BEFORE YOU GO: TIPS FOR MAKING A SUCCESSFUL RESERVATION
• When booking multiple rooms for one organization, please supply the first and last names of each guest using the “Special Requests” dialogue box.
• Let us know if the guests you are sending will have the credit card used at the time of booking, we discourage the use of credit card authorization forms over email.
• Credit card used at the time of booking will be authorized for any unpaid room charges and incidentals unless specified. Incidental authorization is an additional $50 at check in. If incidentals are not covered it’s best if room charges are paid in full prior to arrival.
• A personal credit card is required at check in if incidentals are the responsibility of the guest.
